• abstract, description, formula, drawings (figures) if any (and its copies in English as file .pdf or .doc or .docx)
• the name and address the applicant
• the name and address the authors
• the date priority and priority application number (if priority claimed)
• legalization copy of priority application (if priority claimed)
  if an application requested conventional priority or priority PCT-application be provided for Ukraine State Patent office the legalized copy of the primary (priority) application and its translation into Ukrainian
  
• signed Power of Attorney
