Required Documents/ Information for Design Application Filing:

• print (of file in format .jpg or .gif or .pdf) the main view of design in 3/4 of the external view
• prints (of files in format .jpg or .gif or .pdf) the important details of design
• top view, bottom view, left view, right view, front view, rear view
• explanation of significant functional properties of design or description (and its copies as file in the format .doc or .docx)
• applicant's name and address
• author's name and address
• the date priority and priority application number (if priority claimed)
• legalization copy of priority application (if priority claimed) if an application requested conventional priority the legalized copy of the primary (priority) application and its translation into Ukrainian should be provided for Ukraine State Patent office
• signed Power of Attorney
